REGISTRATION FORM & INSTRUCTIONS

1. Print this page. (Browsers "FILE" menu at the top of the screen, "PRINT" command)
2. Fill out the Name/Address & Event fee section on the registration form, arrive at total cost, and fill in the amount.
3. Make out a check for amount arrived at on form, payable to: 
    Bill & Sandy Van Pool
    If registering for more than two attendees, attach additional forms as needed
    No Refunds after May 15, 2008
4. Send the filled out form(s) and check VIA US mail to:

5. If registration confirmation is requested, enclose a stamped, self addressed envelope. 
    Otherwise pre-registered packets will be available at the door.

7. Don't Forget to make any necessary Tan-Tar-A Room Reservations independently from Event Registration

If you have trouble printing out this form, check your browser page margin settings. You probably have page margins set too large which is cutting off the page.  Try this to correct the problem.  Open Internet Explorer, on the top line menu go to: "File" then "Page Setup", Be sure that page margins are set as follows:  "Top, Left, Right to 0.25", "Bottom to 0.75"  These margin settings should work for most inkjet and laser printers.
